| Patient safety is at the forefront of the concerns of the leaders in health care. Medications contribute to quality of care, safety and enhance the quality of life for patients. However, unless used appropriately, medications can be detrimental to the quality of care and patient safety. The advent of Medicare Part D has stimulated interest in the provision of services to monitor patient safety not only for the elderly patient but also for all patients. A component of the Medicare Modernization Act and Part D is called Medication Therapy Management Services (MTMS). Medication Therapy Management specifically addresses the safe, effective and correct use of medications in patients with multiple diseases or conditions. With the ever changing array of medication therapies available, and the fractionated care of patients, it has become extremely difficult to provide safe and coordinated care with respect to patient medication therapy.
Clinical Pharmacy Systems has developed an internet based application to assist health care providers to meet the patient safety goals. The CPSI Patient Care Manager (PCM) allows the health care provider to manage all of a patient's medication in a centralized location. The PCM gives the provider tools to document therapies being monitored, document the clinical and financial affects of the provided services and document the overall patient clinical progress. The unique aspect of the Patient Care Manager is found in the treatment of the collected data.
